Dear Aviary Homeowner,
All pool cards are deactivated each winter and remain deactivated until this form is returned. In order for us to monitor the authorized use of your pool, we are in need of the information listed below. Please complete and submit this form a minimum of two weeks prior to the pool opening day, so that your card will not remain de-activated. Please check our calendar for pool opening and closing date information for the pool season.
